Evergreen Commons is a community activity center for adults 55 years and older. It offers various health services, such as home visits by nurse and nurse aides; physical, speech and occupational therapies; medical social worker for patients needing intermittent post-hospitalization care; wound care; injections and catheter care. The center also provides personal care services, including assistance with toileting, grooming, dressing, meal preparation, laundry and light housekeeping. It also provides other services, including group and home delivered meals; information and referral; caregiver training and transportation; social services and educational programs. The center offers membership to various clubs that include singles club; camera, stamp and postcard club; and tulip trimmers club. Evergreen Commons is located in Holland, Mich.
