Our mission is to assist client to attain health care goals, while maintaining indepence and dignity, through appropriate home health care and supportive services. We will promote wellness in the community utilizing our experiences, knowledge and resources. CNS is the oldest and only freestanding not-for-profit home health and hospice agency in Utah. All programs are Medicare and Medicaid certified aned we are the only CHAP accredited program in Utah. We are providers for almost all insurance companies. Our staff of registered nurses, therapists, social workers, pharmacists, certified nursing assistants, homemakers, and volunteers offer comprehensive care.
