Three Oaks Hospice provides hospice services to patients in Chicago who are suffering from terminal illnesses We are committed to the comfort, dignity, respect, and support of hospice patients throughout their journey Our hospice team consists of registered nurses, hospice aides, social workers, chaplains, and volunteers We work in close collaboration with patients' families and primary caregivers to ensure that all of the patient's needs are met We are proud to serve the greater Chicago community and look forward to helping more patients and families receive the compassionate care they deserve
