Provided by YP.comThe worst Workers Compensation Doctor. He could not complete a simple calculation and PR4 form. He waited 6 months to decide he could not do the calculation and fill out the form. According to the insurance company, he charges them for my visits. He has cost me quite a bit of money for which I cannot be reimbursed by the insurance company because of time off (sick leave) and travel mileage. I have filed a complaint with the State and will pursue further action if I can.
